The full form of ICSI is Intracytoplasmic Sperm Injection. It is a specialized form of IVF treatment that is primarily used to treat severe male-factor infertility cases. In IVF (In Vitro Fertilisation), a bunch of sperms are placed near the egg to facilitate fertilization naturally. But in the ICSI process, a single sperm is injected directly into the mature egg to facilitate fertilization. ICSI is sometimes done along with IVF to fertilize the egg precisely. How ICSI works?
Due to some reasons, the sperm might not be able to penetrate into the outer layer of the egg or unable to swim to the egg. In such cases, an egg can be fertilized in two ways, either by a traditional process called IVF or by ICSI. More than 50,000 swimming sperms are placed near the egg in the petri dish in a laboratory in the conventional IVF procedure. Fertilization happens when any of the sperm penetrates into the cytoplasm of the egg.
In the intracytoplasmic sperm injection (ICSI) process, a micropipette (tiny needle) is used to precisely inject the selected sperm into the egg. Once fertilization occurs either by traditional IVF or ICSI, the embryo(fertilized egg) kept under observation in a laboratory for around five days and then is transferred into the woman’s uterus (womb).
How is the ICSI procedure performed?
- Like the IVF procedure, fertility drugs will be given to stimulate ovaries to produce more than one egg for fertilization. When it is time, the eggs are collected through an outpatient procedure. The retrieved eggs can be used immediately if the couple is ready or frozen to use later on.
- Meanwhile, the sperms sample from the male partner is collected and washed, and a single healthy sperm is chosen.
- Now the sperm is injected carefully into the centre of the egg with the help of tiny fine needles. This entire ICSI procedure is done under a powerful microscope.
- After the egg is fertilized, it is placed in an incubator. After 24 hrs, the egg shows the signs of fertilization. Slowly the egg continues to form an embryo.
- After a few days, i.e. the embryo develops eight cells, it is ready to be placed in the uterus. The transfer of the embryo is done within five days, which is called Blastocyst transfer.
- You can have more than one embryo transfer. If a single embryo is transferred, it is called elective single embryo transfer, or eSET. Having a blastocyst transfer almost confirms your pregnancy chances. If everything goes well, the transferred embryo gets attached to your uterus wall and starts to grow like a normal pregnancy.
- After two weeks, you can take the pregnancy test for confirmation.
When is the ICSI procedure suggested?
ICSI is suggested if the couple having poor or no fertilization in IVF procedure or in the following conditions:
- Low sperm count
- Poor sperm motility
- Abnormally shaped sperm
- Men who undergone a vasectomy
- Antisperm antibodies that inhibit the sperm function
- Unsuccessful vasectomy reversal
- Unexplained infertility
- Production of a low number of eggs/poor quality eggs
- Using frozen eggs
